Please download and complete all of the New Hire Paperwork by following the instructions below. Once completed, scan and email all PDF copies to officemanager@alanjblair.com

You must schedule a time to stop by the office with your signed I-9 and government-issued ID.

These documents must be completed and received by us one working day before your

temporary assignment is scheduled to start.

Please read this guide that covers your employment through Alan J. Blair Personnel, including our process for timesheets, pay periods, breaks, and other employment information.

You will receive an email from www.hireright.com with a link where you can input your I9 information. Please schedule a time to come into the office to show your identification before your first day.  We must see the original before you start working.

Identification can be;

  • a) Passport (can be expired), or Work Authorization Card/Visa 

or 

  • b) Government-issued current DL or ID Card AND Social Security Card or Birth Certificate.

Please print and sign this document to confirm you are aware you are covered by Alan J. Blair Personnel Services, Inc.'s Worker's Compensation Insurance if you are injured while employed by our company.

Alan J. Blair Personnel Services, Inc. is happy to offer tax-free dollars in your paycheck to be used for your commuting expenses. Please print and complete.

Alan J. Blair Personnel Services, Inc. will pay into the Healthy San Francisco Program for you once you work over 10 hours per week for 3 months.  Complete this form ONLY if you are covered by another person's or company's insurance plan and do not wish to utilize this program.

We are proud to provide a diversified workforce both internally and for our clients. Please print, complete the questionnaire, and return the page with the code to assist us with the Work Opportunity Tax Credit Program. As a small local business, we greatly appreciate it.

Once you email your documents to us you will receive an email inviting you to register with PaychexFlex.com. Click on the link provided and complete your W4 and Direct Deposit before your start date. 

You can view your weekly paystubs or edit your tax and direct deposit information at any given time in your Paychex Flex account.

Please click on the link to complete the state-mandated Sexual Harassment training if you are scheduled to work 100+ hours in your temporary assignment. You must complete the 1-hour training before starting in the position. Once you have completed the training save the certificate at the end of the video and email a copy to officemanager@alanjblair.com. We must receive this certificate before you start in the position. One hour will be added to your first timesheet to pay for this time.
